State of Alaska Medicaid Senior & Disabilities Services Waiver Services

SDS provides medicaid waiver services to children & adults who have been diagnosed with an intellectual developmental disability (IDD) and who reside in urban and rural remote Villages.
An IDD is a severe, chronic disability that is attributable to a mental or physical impairment (or a combination thereof), is manifested before the age of 22, is likely to continue indefinitely, and results in substantial functional limitations in three or more of the following areas of major life activity:
- Self-Care
- Receptive & expressive language
- Learning
- Mobility
- Self-Direction
- Economic self-sufficiency
Forms
Once your child or adult with IDD is found eligible by the State of Alaska SDS Program, they will be placed on a Developmental Disability Registry & Review (DDRR). When individuals are chosen for a medicaid waiver, families will be asked to choose a Care Coordinator, who will help with developing a Plan of Care and arranging for direct care services.
IDD Home Community Based Medicaid Waivers and Individual Support Waivers (ISW) provide a variety of habilitative and non-habilitative services to individuals with IDD. Services include Family Directed Respite Care (at TCC) for families who are primary caregivers who need a break from the constant care of a child or adult with intellectual developmental disabilities. Other direct care services available are Day Habilitation, In-Home Support and Supported Living.
